At Post 16 we understand that moving on from our provision is a significant step for learners and their families. Our transition process is carefully planned and personalised to ensure learners feel confident, supported and well prepared for their next stage. Transition planning begins early and develops over time. We work closely with learners, families, careers advisors, adult services and external providers to explore appropriate pathways and ensure the right support is in place. See below for the various steps we take as part of the transition process.
All families are invited to attend our annual transition event with guest speakers from a range of post 16 and post 18 education providers and adult services. This event provides an overview of future options, key timescales and the support available throughout the transition process.
During the autumn term of the learner’s final year, learners and families meet with a careers advisor from New Directions. This meeting focuses on exploring education, training and employment pathways that reflect the learner’s strengths, interests and EHCP outcomes.
Families and learners are encouraged to visit potential post 18 provisions, including colleges and adult service settings. These visits help learners become familiar with new environments and support informed decision-making.
As learners approach their 18th birthday, families are supported to request a referral to adult services. This helps ensure continuity of support as learners move into adulthood.
An EHCP review meeting is held before or shortly after the Christmas break in the second academic year. Families, professionals, and external providers work together to review progress and agree next steps and exit pathways.
During the spring term of their final year, learners attend taster sessions at their new provision. These sessions help reduce anxiety, build confidence, and support a smooth transition into their next stage of adulthood.
